sent an email with "Credit/Debit Card Authorization"

has all the standard information Name, Customer ID, Phone #, Address.

Under that is an Authorization to charge my CreditCard with $_____

Do I fill this part out since I am not trying to deposit anything?

I am trying to do person to person. Live help person sent me this email said I had to fill that form out to send with drivers license and utility bill copies
