Code blue this morning.
Patient was a 67 year old man with history of obesity (around 350 lbs), hypertension, hyperlipidemia, fatty liver, COPD, insulin dependent diabetes, and admitted with Covid.
He struggled all night on the bipap. Confused, pulling it off, being anxious, etc. He then had a rhythm change and looked to be having a MI. It was finally decided he needed to be put on the ventilator. He was intubated successfully, but he went into vtach. Initial resuscitation efforts were successful, but as happens often, his heart stopped again, and he didn?t make it.
Chest compressions are hard anyway, but when the patient is obese like this, they are near impossible. We had a group of 6 rotating in and out since it is so hard. This guy has covid, so we are all wearing double masks or a PAPR, plastic isolation gowns, and of course gloves. I tapped out after three rounds of two minutes (of compression). When I took off my PPE, I looked like I had jumped in a pool. I was soaked in sweat and exhausted. Your reality as a nurse now are these terrible working conditions. I hate covid.
PS. I got vaccinated today. I was/am scared of it, but I don?t want to end up like this guy if I can prevent it. Guess I?m a pussy. No problems so far 8 hours post vaccination.
